Password Change/Reset Request Form

To have your Digital Locker account password changed, or if you received a "Login Failed" message when attempting to login to Digital Locker, please fill out the form below. Your password will be changed to the password you request within 48 hours.

Please note the following requirements for any requested password:

  • Must be at least 6 characters in length
  • Must not exceed 30 characters
  • Must contain at least 1 lower case letter
  • Must contain at least 1 capital letter
  • Must contain at least 1 number
  • Must contain at least 1 special character
  • Can not contain white space.
