Official Forms and Transcript Requests

REQUESTING A TRANSCRIPT

Official Meredith College transcripts are issued only at the written request or authorization of the student. When making a request, please complete the transcript request form or include the following in your written request:  your full name (including any name you may have used while attending Meredith), social security number, date of birth, dates of attendance, a full legal signature, and the exact address where the transcript is to be sent.

Up to 10 copies of transcripts may be requested (at one time) at no cost. For more than 10 copies, transcripts are $5.00 each.

Requests may be mailed or delivered in person to:

Please allow 3-4 business days for your request to be processed.

Note: If someone other than the student will be picking up the transcripts, please note permission on the transcript request form and include the person's name.

As of October 5, 2009, transcripts can be mailed by or picked up in the Office of the Registrar. Transcript request forms will continue to be accepted via mail, fax or in-person in the Office of the Registrar.
